Please register at Edith Cavell Surgery to continue to your care at Streatham Place Surgery

You were recently sent a letter from NHS South East London CCG, advising you that the contract for GP services at Streatham Place Surgery will end on 30 September 2020 and will become a branch site of Edith Cavell Surgery.

Full services and opening times will be maintained at the Streatham Place Surgery site and the same team of doctors, nurses and reception staff will continue to be based at the current location.

In order to maintain your registration and continue to see your practice team from the current location, you will need fill out a short registration form with Edith Cavell Surgery. This is purely for administrative reasons, because patients currently registered at Streatham Place cannot automatically be transferred to Edith Cavell Surgery. We are sorry for this inconvenience but it is beyond our control.

Streatham Place Surgery and Edith Cavell Surgery have been working very closely together for the past few years. Many of you will have noticed this through the extended access that we have offered to you at the Edith Cavell site on weekends and evenings. This has worked very well, and bringing the two teams together formally under one contract (Edith Cavell) was a natural next step.

What do I need to do now?

**VERY IMPORTANT** – you MUST register with Edith Cavell Surgery by 10th September 2020 if you wish to be seen at the Streatham Place Surgery location by your current practice team, otherwise you may be left without a GP surgery altogether. Please register as soon as possible to ensure there is no break in your care using one the following methods:

  3. By visiting the practice: a member of our team will help you to register

Apart from filling in the form, you do not need to do anything else. Once we receive your completed form, your medical records will be available to the GPs and reception teams across both locations.

We will ensure that this transition will be as smooth as possible for each of our patients.
