Streatham High: Disruption to service on Monday 5 March 2018

Update: 8.00am on Tuesday 6 March 2018: Normal service has resumed

Dear Patients,

We are pleased to inform you that the emergency water works were successful and Gracefield Gardens Health and Social Centre has reopened. Service at Streatham High Practice has resumed as normal this morning.

Update: 9.30am on Monday 5 March 2018: No physical access to Streatham High Practice until further notice

Dear Patients, 

Thames Water have informed us that they are undertaking emergency repairs to burst and leaking water mains in and around the Streatham area. This is due to the recent freezing weather conditions, and is directly affecting Streatham High Practice and all other services within Gracefield Gardens Health and Social Centre.

Therefore, the management of Gracefield Gardens took the decision to close the premises at 9.30am today, so there is currently no physical access to Streatham High Practice until further notice.

We are continuing to deliver care to patients via our nearby practice Edith Cavell Surgery and via telephone appointments where appropriate. Some less urgent needs may be rescheduled.

The telephone number for Streatham High Practice has been diverted to Edith Cavell Surgery temporarily, so you can continue to ring the same number should you need to contact us. 

Finally, we would appreciate if patients only contact the surgery for anything essential whilst the disruption to service continues, thanks in advance for your cooperation and understanding.

 

What to do in an Emergency

When the surgery is closed, please call the Emergency Line on 0208 693 9066 and you will be put through to SELDOC who provide Out of Hours care. This is staffed by local GP’s on a rota basis. You will probably be seen by a GP not from our surgery.

If you need to be seen, you may be asked to attend SELDOC’s Out of Hours Surgery at Dulwich Hospital ,East Dulwich Grove, London SE22.

 

NHS 111 Service

People should use the NHS 111 service if they need help or advice urgently when it’s not a life-threatening situation.

You should call 111 if:

  • it’s not a 999 emergency
  • you don’t think it can wait for an appointment with your GP
  • you don’t know who to call for medical help

For immediate, life-threatening emergencies, patients should dial 999.

Streatham High Practice celebrates 10 years at Gracefield Gardens – Friday 15 December 2017

Streatham High Practice is holding an Open Day to celebrate 10 years of being based in Gracefield Gardens on Friday the 15th December 2017 between 12.00pm and 4.00pm. All are welcome to attend. See poster below.

Visitors will have the opportunity to meet PPG representatives and members of practice staff, as well as being treated to FREE mini health checks and flu jabs for eligible patients. There will also be FREE refreshments and face-painting for our younger visitors.

As well as this, a number of local Healthcare services will be present to offer advice and to answer your questions.

The services due to attend are: 

  • The London Ambulance Service, the busiest emergency ambulance service in the UK. The London Ambulance Service serves over eight million people who live and work in London and employ over 5,000 members of staff. 
  • Carers Hub Lambeth, offering support and advice to unpaid carers.
  • The Lambeth Food Co-Op, developing growing gardens around Lambeth for residents to learn how to grow fresh produce. They also specialise in gardens for GP Practices, for patients, especially those with long-term illnesses, to learn to grow food.
  • Alzheimers Society Lambeth and Southwark, a service that provides support and advice to people who are worried about or affected by dementia.

Streatham Place: Good CQC Rating

CQC’s new programme of inspections of England based GP practices focuses on rating according to whether they are safe, effective, caring, responsive and well led. Inspectors rated Streatham Place Surgery overall “Good” in all domains with Outstanding features.

Professor Steve Field, Chief Inspector of General Practice said “We carried out an announced comprehensive inspection at Streatham Place Surgery on 7 June 2017. Overall the practice is rated as good.

Our key findings across all the areas we inspected were as follows:

  • There was an open and transparent approach to safety and a system in place for reporting and recording significant events.
  • The practice had clearly defined and embedded systems to minimise risks to patient safety.
  • Staff were aware of current evidence based guidance. Staff had been trained to provide them with the skills and knowledge to deliver effective care and treatment.
  • Results from the national GP patient survey showed that in most areas patients were treated with compassion, dignity and respect and were involved in their care and decisions about their treatment.
  • Information about services and how to complain was available. Improvements were made to the quality of care as a result of complaints and concerns.
  • Patients we spoke with said they found it easy to make an appointment with a named GP and there was continuity of care, with urgent appointments available the same day.
  • The practice had good facilities and was well equipped to treat patients and meet their needs.
  • There was a clear leadership structure and staff felt supported by management. The practice proactively sought feedback from staff and patients, which it acted on.
  • The provider was aware of the requirements of the duty of candour. Examples we reviewed showed the practice complied with these requirements.

We saw several areas of outstanding practice:

  • The practice had invested in a business intelligence tool which provided ready access to searches relevant to medicines management and effective care outside of those provided through the patient record system. This provided ready access to information relating to when follow up tests for patients on medicines used to manage long term condition and patients with poor mental health. All staff were involved in the monitoring and improvement of outcomes, and there were both clinical and administrative leads in place to ensure that follow ups were scheduled. The practice had evidence to show that the system had improved compliance of tests in the eighteen months that they had managed the practice.
  • The provider had arranged relevant in house training for all staff in the practice for the next year. Training was determined on the basis of role, and training was delivered in person, by Webex and where necessary 1:1. For clinical staff this focussed on the management of long term conditions and mental health. All staff at the practice undertook training on the Mental Capacity Act and Deprivation of Liberty Safeguards.
